ChosenOneSports Service Agreement
This agreement between ChosenOneSports & applicant _____________________ gives ChosenOneSports permission to create an athletic profile and forward/contact College recruiters on their behalf in pursuit of athletic recognition, athletic scholarships & financial aid. In compliance with NCAA/NAIA stipulations, applicant /acknowledges that ChosenOneSports, nor any of its representatives promised the awarding of athletic/academic scholarships or financial aid of any kind{pursuiant to NCAA regulations) nor did ChosenOneSports or any of its information providers, employees, representatives or officers make any warranty/guarantee as to the results to be obtained from the usage of this service. Applicant agrees to provide accurate, complete and current information in the registration which includes all information requested in original sign up. Applicant assumes all responsibility for the arrangement & shipping off of all highlight films & tapes. Also, applicant grants Chosen One Sports permission to disclose certain registration information to certain 3rd parties (Colleges/Universities & their representatives) in pursuit of athletic/academic scholarships & financial aid. Applicant acknowledge that once service is subscribed to, no refunds will be issued. *(Student athletes that are placed into Jr.Colleges/4yr. programs from utilizing our service that may require future use for 4yr institutions after expiration of Jr.College eligibility and/or for transfer purposes may be charged an additional fee (at that time) at a reduced rate to offset overhead and expenses).
